Before Your Appointment

To help make your visit as efficient as possible, we encourage all new patients to complete their registration forms prior to their appointment. Completing your paperwork in advance allows our team to review your medical and dental history and prepare for your consultation.

Please bring the following items with you:

  • A valid photo ID
  • Your dental insurance card(s), if applicable
  • Your medical insurance card
  • A list of current medications
  • Referral information from your dentist, if applicable
  • Any recent dental x-rays or imaging that may assist with your evaluation

Your First Visit

Your initial appointment will include a review of your medical and dental history, a comprehensive examination, and a discussion of your concerns and treatment goals. Depending on your individual needs, digital imaging and additional diagnostic records may be recommended to assist in developing a personalized treatment plan.

Our doctors take the time to explain findings, answer questions, and discuss all appropriate treatment options so that you can make informed decisions regarding your care.
